NPS DDO Registration Form

Instructions for filling the form:

1. The form should be filled up completely. All (*) fields are mandatory.
2. Mention the DDO Code in relevant field, if allotted by the Treasury Officer / PAO.
3. e-mail ID should be the official e-mail ID of the Drawing and Disbursing Officer and not of personnel.
4. The form is to be submitted to 'NPS' Cell, O/o the Director of Treasuries and Accounts, Sri Anjaneya Towers, C-Block, II Floor, VTPS Road, Ibrahimpatnam - 521 456 with covering letter.
