APCOS Intimation of Death(Accidents/Accidentally) of Outsourced Manpower to APCOS office -Reg.

Request all DDO'S to send death intimation of their Outsourced Manpower if any to APCOS within 24 hours, Otherwise APCOS will not be liable for rejection of benefits received after due date.
Delayed submission of Accident Reports often effects the investigation adversely resulting in denial of benefits to the beneficiaries. Moreover, non submission or delayed submission is violation of provisions of ESI Act which attracts legal action.
It is, therefore, requested to furnish such information of accidents, especially fatal accidents, within 24 hours of the occurrence of accident, so that Investigations can be conducted immediately. Further, it is to inform that all the documentary evidences can be furnished at later stages, but, informing an accident immediately should be the top priority. You are advised to bring the above contents to all your field offices in the best interest of the effected Insured Persons.
Following documents are mandatorily required for processing claims:
1) Department covering letter
2) Death Certificate in original
3) Family member certificate
4) Aadhar of deceased Insured Person
5) Discharge summary/Death summary
6) FIR Copy
7) Postpartum report/Seva panchanama
8) Manual Attendance of deceased IP (9 months/upto death date)
9) 2 persons Witness with self letter, Aadhar cards and mobile number
